29 CFR 1910.178(q)(5):Parts of powered industrial trucks requiring replacement were not replaced with parts equivalent in safety with those used in the original design. (a) On or about 5/27/2021, 1901 13th Avenue East, Tuscaloosa, AL 35404, in the stockroom area - employees using Work Assist Vehicle (WAVE) platform were exposed to fall hazards up to 17 feet to the concrete floor because the gate was repaired using duct tape and a liquid weld epoxy and not with parts equivalent in safety with those used in the original design.
