29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Florentino Velasco located at 2384 Vineyard Road, Columbus, OH 43220: On and preceding May 25, 2021, the employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Roofers were installing weather seal on a roof that varied from 10-28 feet. The employees were not utilizing proper fall protection which exposed them to falls up to 28 feet. Florentino Velasco was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1501652, citation number 1, item number 3 and was affirmed as a final order on April 12, 2020, with respect to a workplace located at 243 Winter Shadow Loop Delaware, OH 43015. Florentino Velasco was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1477518, citation number 1, item number 3 and was affirmed as a final order on July 23, 2020, with respect to a workplace located at 551 Buchloe Dr., Lot 6900, Marysville, OH 43040. Florentino Velasco was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1412816, citation number 1, item number 5 and was affirmed as a final order on September 3, 2019, with respect to a workplace located at 5135 Tarlton Blvd., Lot 7500, Hilliard, OH 43026.
