29 CFR 1910.333(a)(1): Live parts to which an employee would be exposed were not deenergized before the employee worked on or near them, unless the employer could demonstrate that deenergizing introduced additional or increased hazards or was infeasible due to equipment design or operational limitations: (a) On or about 05/09/2021 - Old Mono Department, lockout/tagout procedures were not developed and utilized by employees changing out 480-Volt fuses on the circuit breaker, exposing employees to arc flash hazards.
