29 CFR 1910.119(f)(4): The employer did not implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel. These safe work practices shall apply to employees and contractor employees. In the Alternative 29 CFR 1910.119(k)(2): The hot work permit did not identify the objects on which hot work is to be performed. On or about May 4, 2021, in the HF Alkylation 3 Unit, the employer did not implement its PR-3 Safe Work Permit Procedure to update hot work permit #1187389 when the work scope was changed to remove additional Primary Isolation Point (PIP) blinds by contractor on the Acid Dump Drums (V-1004 and V-1051).
