29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: a) Facility, 4810 Buttermilk Hollow Road, West Mifflin, PA: On April 6, 2021, an employee was exposed to and suffered a fatal fall due to slag that had covered the walkway that he used to perform maintenance activities and had also accumulated into a large pile that he had to climb on/adjacent to to reach an area to grease.
