29 CFR 1910.28(b)(6)(i): The employer did not ensure that each employee less than 4 feet (1.2 m) above dangerous equipment was protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system. The equipment was not covered or guarded to eliminate the hazard. (a) In the Forming Line Room, on or about 3/23/2021: the meat forming hopper on the Provatec 760 forming machine (serial #760) was not protected to prevent employees from falling into the rotating augers within the hopper. Employees were exposed to amputation hazards due to rotating augers located 22" below the lip of the hopper edge.
