29 CFR 1926.451(a)(1):Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it. a. 136 Summit Avenue Jersey City, NJ Employees performing masonry work from a job-made elevated work platform constructed over an elevator shaft were exposed to a fall hazard of approximately 97ft when the platform was unable to support the applied load of materials and personnel without failure. On 3/10/2021, the elevated work platform partially collapsed while two employees were working from it. One employee fell approximately 97ft to the bottom of the elevator shaft, sustaining fatal injuries. Violation occurred on or about 03/10/2021
