29 CFR 1910.146(c)(2):The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces. Location: a) Inside elevator motor room of passenger elevator, ID: 3P871: On or about March 5, 2021, an employee entered an elevator motor room to retrieve a dropped phone. The room was a permit-required confined space, and the employer did not post danger signs or other equally effective means to inform employees of the existence and location of the permit space, and the hazards presented by the elevator equipment.
