29 CFR 1926.651(g)(1)(iii): Adequate precautions were not taken such as providing ventilation, to prevent employee exposure to an atmosphere containing a concentration of a flammable gas in excess of 20 percent of the lower flammable limit of the gas. On or about March 05, 2021, at the 18700 Block of Gleannloch Lakes Boulevard in Spring TX, six employees were exposed to explosion and fire hazards while repairing a gas line leak without ventilation or other precautions in an atmosphere containing a concentration of a flammable gas about the lower flammable limit of the gas.
