29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Startup Cyclone. On or about February 21, 2021, employees performing servicing and maintenance, including but not limited to, clearing a clog in the cyclone, did not follow the energy control (lockout/tagout) procedures before entering the space below the cyclone to clear the clog. Employees received burn injuries when steam (thermal energy) escaped during the unclogging operation.
