29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily: On or about February 3, 2021, in the Activator Production workroom by Activator #3, the valve on the natural gas line for Activator #4 that was taken out of service did not have a warning tag for employees not to open it.
