A) Custom Bandag, Inc.-8181 Route 130 South, Pennsauken, NJ: The employees utilized a Toyota PIT that was not maintained in such a condition that all nameplates and markings were in a legible condition. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. Condition noted on or about 1/20/2021
