29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer is failing to protect the employees from struck-by and crushed-by hazards. This was most recently documented on or about January 14, 2021, when the employer did not ensure that a lockout device was applied to the 480 volt electrical energy source associated with the Okura Robotic Palletizer while an employee was working inside of the robotic cell cage.
