29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about December 28, 2020 and times prior thereto, the employer failed to ensure the lockout/tagout written procedures had clear steps to shut down and isolate energy sources when employees engage in maintenance or operational type activities, exposing employees to amputation hazards.
