29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Warehouse Dock Area - On or about 12/16/2020, the employer exposed employees to struck-by and crushing hazards, in that employees, untrained in their native language, were authorized to operate the Toyota diesel sit-down fork truck (Model # 8FDU30 S/N #10088).
