29 CFR 1910.146(f)(7): The entry permit that documented compliance and authorized entry to a permit space did not identify the hazards of the permit space to be entered: (a) On or about 5/4/2021, and at times prior - Fuel House - Upper (a.k.a. - Tee Pee), employees were exposed to engulfment hazards from bridged and normal sawdust because the employer had not identified this hazard nor listed it on the authorized entry permit for the space.
