29 CFR 1926.602(c)(1)(ii): No modifications or additions which affect the capacity or safe operation of the equipment shall be made without the manufacturer's written approval. If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. In no case shall the original safety factor of the equipment be reduced. a. Located at 4172 Holstein Drive in Columbus, Ohio: On or about December 9, 2020, employees were exposed to struck-by hazards as a result of an unsecured fork extension attached to a Pettibone rough terrain forklift with a wire rope sling free rigged to the extension to hoist roof trusses. The addition to the forklift was not approved by the lift manufacturer, thereby affecting the safe use of the lift.
