29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck At the workplace located at 10221 NW Transcon Drive, Kansas City, Missouri: Employees were exposed to struck by and amputation hazards as at least two temporary employees were not completely trained on operating push pull forklifts. At least two forklift operators had not received formal and practical training, and evaluation of the forklift operator's performance in the workplace prior to the employee operating the push pull Hyster forklift. On or about November 18, 2020, a temporary employee from one company received an amputation injury when assisting another temporary employee from a second company to grab a slip sheet with the push pull attachment being used with a Hyster forklift.
