29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a.) Production, Line 12 - On or about November 14, 2020, the employer failed to provide training to each authorized employee performing service and/or maintenance on equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. An extrusion operator helper (EOH) sustained the partial amputation of two (2) fingers while inspecting the Line 12 BHA S112 duct collector.
