29 CFR 1926.502(d)(8):Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: Bay 5 of the Formation Building located at 1760 Steve Reynolds Pkwy, Commerce, GA: On or about November 4, 2020, employees were exposed to fall hazards up to 52 feet, when more than two employees were simultaneously connected to a lifeline that was rated for the use by two or fewer employees.
