29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Facility: The energy control procedures were not documented for machines including, Apex Automatic Four Head Gravity Filler, Autolabe Label Applicator, and REB SVS Piston Filler, on which servicing and/or maintenance is being performed by the employees under lock-out/tag-out, on or about 10/30/2020. b) Facility/Production Area: The energy control procedure was not utilized when employees were required to clean the conveyor belt of Apex Automatic Four Head Gravity Filler where an in-running nip point danger zone existed during a machine operating cycle, on or about 10/30/2020.
