29 CFR 1910.28(b)(15):Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level was protected from falling. On or about October 2, 2020 and times prior thereto, the employer did not ensure that employees accessing the top of the glass crusher in the 12-Mesh Building were provided fall protection and a safe work platform free from exposing employees to fall hazards of approximately seven (7) feet.
