29 CFR 1926.651(k)(1):Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: On or about September 21, 2020, at an excavation site located at the intersection of Lincoln and Santa Fe in Carney, OK, a competent person did not conduct an inspection of an excavation to ensure conditions did not exist that could result in possible cave-ins, exposing employees to struck-by and crushed-by hazards.
