29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a) Miguel Dominguez, at a jobsite located at 768 Wharncliff, Lot WEL138, Pickerington, Ohio 43062: On and/or preceding October 20, 2020, the employer did not ensure employees who used extension cords were protected from electrical contact hazards. Roofers were using a yellow extension cord with the ground prong missing from the male end of a cord, which exposed the employees to a 123 volt electrical contact hazard.
