29 CFR 1910.304(g)(5):The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective. On or about 23 October 2020, employees were using a pedestal fan, serial # 005637, that was missing the grounding pin on the power cord plug, exposing the employees to an electrical shock hazard.
