29 CFR 1910.147(f)(3)(ii)(D): A personal lockout or tagout device(s) was not affixed to the group lockout device, group lockbox or comparable mechanism when he or she began work (and to be removed when work stops) on the machine or equipment serviced and/ or maintained. (a) On or about September 17, 2020, at the #5 Crude unit of the Calcasieu Refinery facility, crewmembers failed to affix a personal lock to the group lockbox before vacuuming leaking crude oil from the heater (H-501) convection section.
