29 CFR 1904.39(a)(1): Within eight (8) hours after the death of an employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. Texstar Enterprises, LLC located at 10 FM 2252, Schertz, TX 78154 On or about June 19, 2020, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within eight (8) hours. The employer reported the death to OSHA on September 8, 2020.
