29 CFR 1910.219(c)(4): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves: In the South Mill Boiler Room, employees engaged in milling activities were exposed to a caught by hazard in that an exposed 4.5 inch diameter shaft end projected approximately 6.5 inches and was unguarded.
