29 CFR 1910.178(m)(4): Industrial truck operators were not prohibited from placing extremities outside the running lines of the truck. (a) A & J Landscaping, LLC, located in Sylvania, OH: On or about August 18, 2020: The employer failed to ensure an employee operating the Toyota 2500 forklift maintained a position inside the running lines of unit while the unit was in a operational condition. The employee, operating from a position between the mast and outside frame of the vehicle was exposed to crushed-by hazards.
