The employer did not ensure that the rope descent system has proper rigging, including anchorages and tiebacks, with particular emphasis on providing tiebacks when counterweights, cornice hooks, or similar non-permanent anchorages are used. On or about August 14, 2020, while in the process of completing window-washing operations, employees who used a rope descent system that was not properly rigged were exposed to a fall hazards of approximately 96 feet. The employer failed to ensure that the employee was anchored before attempting a descent off an eight (8) story building.
