29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about July 13, 2020 the employer failed to ensure that operators of the extrusion lines were trained as authorized employees for the control of hazardous energies and LO/TO program. Operators of the extrusion lines are tasked with and responsible for changeovers and setups on the equipment without isolating the hazardous energy sources. Hazardous energies present on the equipment include but are not limited to pneumatic, mechanical, thermal, and electrical.
