29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: On or about 7/23/2020, throughout the facility, workers were exposed to burns and explosion hazards where the use of non-intrinsically safe inspection lighting, such as but not limited to flashlights and cell phones, were used to inspect chemical levels inside of mixing vessels. Chemicals being mixed included but were not limited to Isopropyl Alcohol.
