Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 1) On July 27, 2020, and at prior times, employees were exposed to the hazard of unexpected start up or release of stored energy when removing an access plate to clear jams in the rotary feeder on Silo #3. The procedure presented hazards from multiple sources of energy and the employer had not developed and documented written procedures for shutting down, isolating, or blocking the machine prior to maintenance and service activity.
