29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) On or before 06/30/20, at the Jamestown Containers facility, located at 82 Edward Deming Drive, in Rochester, New York, the employer did not prevent an employee from falling 30 feet to a lower level through a roof opening by covering the hole, barricading the hole or providing the employee with a personal fall arrest system.
