29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: a) Outside of the Warehouse: An employee was operating a Clark Forklift, Model CGC25 /Serial Number / C365L / 0143- 9538FB, with a job-made forklift jib boom attached. The job-made attachment was added to and used on a Clark forklift without the prior written approval of the forklift manufacturer; on or about June 3, 2020. Note: In addition to abatement certification, the employer must submit abatement documentation for this item in accordance with 29 CFR 1903.19.
