29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation. Employees were exposed to struck-by hazards at the Grainfield, KS location. Employees engaged in material handling activities using the Caterpillar Telehandler, Serial #TBN00750 were exposed to struck-by and caught in-between hazards in that comprehensive forklift training was not provided prior to permitting forklift operations.
