29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: RAM Construction Services of Michigan, Inc. - On or about March 21, 2020 at the worksite located at 226 Middle Ave. in Elyria, Ohio: The employer failed to indicate to employees that a floor hole was present on the structure's second level. Employees were exposed to a ten-foot fall hazard in that the plywood, used to cover a 3' x 7' floor hole, was not marked to warn employees of the hazard; the cover was moved and an employee sustained fatal injuries after falling through the hole.
