29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about March 13, 2020 and times prior; the #69 110 tom press, serial number 54-3196, Dorsey Fixed Asset number 71-11514: The employer exposed an employee to the hazards associated with point-of-operation (POO) contact with the #69 press by allowing him to work at/near the point-of-operation area while positioning and removing parts from the die during production. The POO is protected by two hand trip buttons, which the left trip button had been defeated by placing a piece of metal banding into it which allowed the operator to only push the right trip button to operated the press.
