29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about March 5, 2020, during the installation of roofing material on the roof of a residential single family home located at 7947 Chatham Ave NW, N. Canton, Ohio 44720; the employer did not keep walking/working areas free of debris that created trip/slip/fall hazards at the base of the portable ladder where employees were accessing and or egressing elevated working heights.
