29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) The Williams Companies, at 23759 Weld County Road 34.5, La Salle, CO: On and preceding 3/2/2020, The Williams Companies did not ensure that procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. Employees performed pigging operations on gas pipelines. Pigging was performed to clean and maintain gas pipelines entering and exiting the facility. The flammable hydrocarbon liquids, vapors and gases in the piping were chemical and thermal energy sources. Employees performing this maintenance activity operated valves of the pipelines, isolating the chemical and thermal energies of the flammable gas stream, prior to retrieving or launching pigs. The employer did not ensure that procedures were developed, documented, and utilized for the control of the hazardous chemical and thermal energies present for receiving and for launching pigs. This condition exposed employees to uncontrolled hazardous thermal and chemical energy hazards.
