29 CFR 1910.132(e): Defective and damaged equipment. Defective or damaged personal protective equipment shall not be used. a) Warehouse - Interior, South Row 36- Hi Lo Order Picker- (Serial Number) 540 - 12- C13715 - An employee was exposed to fall hazards through an inadequate fall arrest system that had a damaged connector. The carabiner connecting the harness to the lanyard had a damaged gate that did not automatically close, on or about March 2, 2020. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
