29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. At the workplace located at 127 Claxton Avenue, Carthage, Missouri 64836, employees were exposed to serious caught-in, struck-by, and amputation hazards associated with servicing and maintaining equipment with hazardous stored energy. An examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.
