29 CFR 1910.146(c)(8)(iv): When both host employer personnel and contractor personnel will be working in or near permit spaces, the host employer did not coordinate entry operations with the contractor as required by paragraph (d)(11) of this section: On, or about February 19, 2020, at the location of 8101 Union Ave., CLEVELAND, OH, 44105, in the hazardous materials bay (8), the host employer, Kenan Advantage Group, Inc. failed to coordinate entry activities with its contractor, Great Lakes Tank & Vessel, LLC (GLTV) when a manager accessed the top of chemical trailer #171128 to observe and communicate with a (GLTV) entrant about the contents of the tanker.
