29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: On or about 2/6/2020, at west side mill of Sugar Cane Growers Cooperative of Florida, 1500 George Wedgworth Way, Belle Glade, FL 33430, an employee was exposed to tripping and stepping into hazards while permitted to do housekeeping near and around a floor hole used for drainage that was not entirely covered.
