29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. At a worksite located at 17562 Gum Rd, Carthage, MO: Employees were exposed to possible toxic and or hazardous substances in that the employer was not following recognized and generally accepted good engineering practices (RAGAGEP): b) This was demonstrated on January 28, 2020, and at times prior thereto, the employer did not document that valve labeling complied with RAGAGEP in that, valve "XV-020" marked on P&ID PID-DNNA-CAMO-EMUL.0012 was not labeled in the field. c) This was demonstrated on January 28, 2020, and at times prior thereto, the employer did not document that valve labeling complied with RAGAGEP in that, valve "SN-009" marked on P&ID PID-DNNA-CAMO-EMUL.0012 was not labeled in the field. d) This was demonstrated on January 28, 2020, and at times prior thereto, the employer did not document that valve labeling complied with RAGAGEP in that, valve "XV-189" marked on P&ID PIDC-CPPR-3148-7 was not labeled in the field. e) This was demonstrated on January 29, 2020, and at times prior thereto, the employer did not document that valve labeling complied with RAGAGEP in that, reactor tank mixer "A-400" marked on P&ID PIDC-CPOT-3155-4 was not labeled in the field. f) This was demonstrated on January 29, 2020, and at times prior thereto, the employer did not document that valve labeling complied with RAGAGEP in that, premaster pad "DP-001" marked on P&ID PIDC-CPOT-3155-4 was not labeled in the field. g) This was demonstrated on January 29, 2020, and at times prior thereto, the employer did not document that valve labeling complied with RAGAGEP in that, valve "XV-200" marked on P&ID PIDC-CPOT-3155-1 was not labeled in the field. h) This was demonstrated on January 29, 2020, and at times prior thereto, the employer did not document that valve labeling complied with RAGAGEP in that, valve "XV-210" marked on P&ID PIDC-CPOT-3155-1 was not labeled in the field.
