29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) On or about 01/23/2020 - 2000 John D Long Drive, SE, Hartselle, AL, employees were exposed to a struck-by hazard when employees who operate forklift trucks had not completed the required formal instruction, practical training, and evaluation of the operator's performance in the workplace.
