29 CFR 1910.132(d)(1)(i): The employer did not assess the workplace to determine if hazards were present, or likely to be present, and did not select and have each affected employee use the types of Personal Protective Equipment (PPE) that would protect the affected employee from the hazards identified in the workplace hazard assessment: On or around January 15, 2020 at the Phillips 66 refinery in Sweeny, Texas the employer did not assess the hazards of opening equipment in the Main Fractionator and Overhead System and exposed workers to hazards when it did not select and have affected workers use Personal Protective Equipment. a) The employer did not evaluate the opening of the 95-200 Fractionator vent for hazards and did not select and have workers utilize PPE to protect against hazards of opening process equipment. b) The employer did not evaluate the opening of the Fractionator Overhead System 95-12 Reflux Drum vent for hazards and did not select and have workers utilize PPE to protect against hazards of opening process equipment. c) The employer did not evaluate the opening of the Fractionator Overhead System 95-31 OH Accumulator vent for hazards and did not select and have workers utilize PPE to protect against hazards of opening process equipment.
