29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) At the demolition, on or about January 6, 2020, and at times prior to, employees were exposed to crushed-by hazards when the employer did not provide employees with training that included procedures for the safe dismantling and demolition of an industrial complex including equipment such as, but not limited to, large filter presses.
