1910.119(e)(3)(v) Facility siting; 29 CFR 1910.119(e)(3): The process hazard analysis did not address the elements of (i) through (vii) of this paragraph: On or about February 19th, 2020, the employer did not have a process hazard analysis (PHA) that addressed the elements required by sections (i) through (vii) of this paragraph. The initial PHA and subsequent PHA re-validations did not adequately address the following elements: a) The identification of any previous incident which had a likely potential for catastrophic consequences in the workplace; b) Consequences of failure of engineering and administrative controls, and; c) Facility siting.
