29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about 12/16/2019, in the pillow production lines one and two at 1720 GA Highway 138, Conyers, GA: Employees engaged in service and maintenance activities on the two picker equipment while clearing, unjamming, or troubleshooting were exposed to amputation and caught-in hazards, in that they did not apply a lock and or tag device on the equipment energy sources. Pickers lines one and two with electrical 240 volts and mechanical moving energy sources.
